We know that good self-management for diabetes is the key to health, well-being and prevention of complications associated with uncontrolled blood glucose levels. Whether a patient is newly diagnosed or has been dealing with diabetes for many years, our Diabetes Self-Management Education Program* provides excellent educational services (group and individual) for people who have been diagnosed with diabetes -- type 1, type 2, or gestational diabetes.

Since we were established in 2001, we have obtained and maintained our American Diabetes Association (ADA) Education Program Recognition Certificate. The Certificate confirms that the Program meets National Standards, delivering high-quality education, an essential component of effective diabetes treatment. Our latest status will remain effective through 2023. All of our educators are Certified Diabetes Educators – registered nurses and registered dietitians who meet strict guidelines and are recertified every five years through testing or continuing education credits in order to remain current with new developments in diabetes care.

For all patients, our Diabetes Self-Management Education Program* begins with a one-on-one assessment visit. Based on the patient’s individual needs, we provide the following types of education either individually or in a group setting, including core content as outlined by the National Standards for Diabetes Self Management Education: 

  • Diabetes overview
  • Nutrition
  • Medication management
  • Exercise
  • Blood glucose monitoring
  • Acute and chronic complications
  • Prevention
  • Stress management
  • Insulin injection instruction
  • Insulin pump initiation training and follow up
  • Continuous glucose sensor initiation training and follow up

After the end of the program, we follow up with our participants; we check in to provide continued support to patients in making lifestyle changes, to see if they are meeting their goals, and to see if they have questions.

Pre-diabetes And A Healthier Lifestyle

For patients who have been told they have pre-diabetes, or who simply want to improve their lifestyle, Stamford Hospital offers individual education and counseling sessions with registered dietitians at the Diabetes and Endocrine Center and the Tully Health Center. At the first one-hour meeting, one of our expert registered dietitians will review the patient’s diet history and help devise an individualized meal and exercise plan tailored to his or her needs and preferences. Follow-up appointments may include a weigh-in.
