I understand that this may mean use of my photo and information about my participation in a Stamford Health event in Stamford Health publications, advertisements (print, billboard, newspaper, online ads, etc.), digital channels (website, social media, email marketing), videos (YouTube, Social, web), on television, or in newspaper or magazine articles.

I release and forever discharge Stamford Health, its officers, directors, affiliates, subsidiaries, employees, agents and successors (“Releasees”) from any and all claims, costs, liabilities or expenses of any kind including, without limitation, damage claims for invasion of privacy, defamation, libel or slander, including without limitation the copyright and/or use, re-use and publication thereof in which I may be distorted in character or form, or reproductions thereof in color or black and white. I hereby waive any right to inspect or approve the finished materials as noted above or advertising copy or printed matter that may be used in conjunction therewith or to the eventual use that it might be applied.
