Stamford Health is thrilled to be able to offer MyChart functionality to our patients. Utilizing MyChart, an electronic health record, will help our providers communicate with you, the patient, more effectively. It allows for seamless integration between providers within our network, quicker interactions with your clinical care team, and more educational health resources for you.

How To Enroll
There are several different methods of MyChart sign up that might be used by different departments across your health care organization:
- You can self-sign up online to create a MyChart account by matching your information against what is on file in your medical record or with third-party identity verification.
- Clinical staff might sign you up directly while you're at the front desk or in the exam room.
- You might receive a MyChart activation code on your After Visit Summary or on a billing statement.
- You might receive a text message or email with an activation code when you come in for a visit. If you lose your activation code or it expired, you can request one by visiting MyChart.

Access Your Previous Patient Portal Account
Linking All Your MyChart Accounts
If you have a MyChart account with another health care system, you will be able to link the accounts so you can easily toggle between the various health care systems. Please watch the video to understand how to easily link your accounts.

How can you access the Patient Portals that Stamford Health previously used?These former Patient Portals will be available for a limited period. When you access these portals after March 1, your information will be in “Read Only” format – you will not be able to interact with your providers or make appointments on your previous accounts.
As we transition all your health data to MyChart, please note some results and past appointment information may only be available by logging into your previous Patient Portal. All your health data from March 1, 2025, onward will be in MyChart. What can you do in MyChart?MyChart offers personalized and secure online access to your medical records. It enables you to manage and receive information about your health. With MyChart, you can:
- Schedule medical appointments.
- View your health information, including medications, allergies, test results, and more.
- Request medication refills.
- Access resources for trusted health information.
- Message your care team.

What does proxy access mean and how do you set it up?Proxy access allows a family member or care partner (a friend, family member, or caregiver involved with a patient’s care) to securely communicate with a patient’s physician’s office. Proxy access is requested from the MyChart homepage. The family member or care partner will need to create their own MyChart account (whether they are a Stamford Health patient or not) then complete the electronic “Request for Proxy Access” form (linked from the log-in page of MyChart) which will be submitted to Stamford Health’s Health Information Management team to review and process as soon as possible.

When can you see test results in MyChart?With MyChart, you can view most test results as soon as they become available. Know that you will likely see results before your health care provider has had a chance to review them. After your provider reviews your results, you might see additional comments and interpretation in MyChart.
