We encourage you to use our Patient Portals as a convenient, secure, and confidential online tool for you and your family to access and manage your health information. Note: you may have multiple Stamford Health or Stamford Health Medical Group Patient Portals depending upon where you received treatment. If you’re not sure which Patient Portal is right for you and the service you received, contact our Health Information Management Department at 203.276.7034, Monday – Friday 8:00am – 5:00pm.

How To Enroll

  • STAMFORD HOSPITAL

    Patient Portal

    The Stamford Hospital Patient Portal allows you to view your personal profile, check test results, request appointments, message providers, request prescription renewals, and view discharge instructions. Successful self-enrollment will only be achieved if you have your Medical Record Number and you enter it as the example shown below.  Additionally the information must match exactly to what is currently on file.

    Easily and securely access your health information from your smartphone through MEDITECH MHealth. 

    After downloading the app, make sure your phone’s location services are turned on so the app can show you the nearest patient portal – Stamford Hospital.

    How to Enroll in the Stamford Health Medical Group Patient Portal

    You must visit your physician’s office in person to enroll in the Stamford Health Medical Group Patient Portal. Please call for office hours. To find out if your physician is part of the Stamford Health Medical Group, go to our Find a Doctor search.

  • REQUEST PROXY ACCESS

    Minor proxy access allows a parent or guardian to access a patient who is under the age of 18. The proxy access becomes more limited on the child's 13th birthday and is automatically terminated on the child's 18th birthday.

    Adult proxy access allows you to access a patient who is 18 years of age or older. To obtain access, both the patient and proxy must complete and sign the Adult Proxy form. The proxy's access is terminated when the patient makes a written request to terminate access.

ENSURING TIMELY ACCESS TO YOUR INFORMATION

As of April 5, 2021, the 21st Century Cures Act mandates that every healthcare provider make test results available without delay. This means you may see your results or care findings before your health care provider has reviewed them. Please allow time for your health care provider to review the results. Please make sure only appropriate individuals have access to your portal through the proxy access. If you have a medical concern or question, please contact your health care provider directly.
