Stamford Health is committed to providing expert care with a human touch for every patient, 24/7. As a Planetree Gold-designated organization, our priority is delivering a person-centered experience that includes the voice of our patients in all that we do. Your feedback is important to us, and to reach as many patients as possible, we utilize a third-party vendor to provide post-visit surveys. These surveys measure all aspects of the patient experience. In the spirit of transparency, and to help patients make informed decisions about their health care providers, we now share our providers’ patient experience ratings online for review.

Who receives the survey?

Medical practice patients, with confirmed email addresses, receive a survey within a few days following their visit. Patients complete the survey online and comment on specific aspects of care. We use this feedback for improvement purposes and to provide each patient with an exceptional experience of care. In consideration of our patients, we will only send a survey once every 90 days regardless of the number of clinic visits.

Do you post all comments?

We are committed to posting unedited feedback, both positive and negative. However, we do not post comments that: (1) include identifiable personal information; (2) include feedback about another provider or department beyond that visit; (3) contain defamatory, profane, racist, or objectively offensive language or (4) comments that contain information which is the subject of an ongoing quality review, or comments that raise other unique concerns.
