e-notification Requestor Portal Requirements

Stamford Health has chosen PatientPing to facilitate the delivery of real-time event notifications for care events occurring at its facilities, as required by CMS’ electronic event notifications Condition of Participation (e-notifications CoP)*.

Once you have submitted your information, a PatientPing representative will contact you directly to assist you in enabling real-time, e-notifications from Stamford Health.

CMS E-notifications Condition of Participation

On May 1, 2020 the Center of Medicare and Medicaid Services (CMS) published the Interoperability and Patient Access Final Rule, which includes a new CMS Condition of Participation that requires hospitals to send real-time electronic notifications to community providers to support patient care coordination and quality improvement. Real-time notifications must be sent to patients’ primary care providers (PCPs), primary care practices/entities, and post-acute (PAC) providers when they are registered/admitted, discharged, or transferred from an emergency department or inpatient service.

Compliance & Notification Infrastructure

Stamford Health has partnered with PatientPing to provide real-time e-notifications upon patient admit, discharge, and transfer events to community provider recipients in accordance with the new CMS requirements. Community providers will be able access event notifications from Stamford Health in two different ways:

Direct Messaging

Notifications will be sent to PCPs and other providers identified by the patient and recorded in their patient record during a hospital encounter via Direct Messages, a national encryption standard for secure clinical data exchange. Providers will have the option to unsubscribe from receiving Direct Messages via a link in the e-notification if they wish.

Notification Subscription Portal

Primary care entities, PACs, and other providers can also proactively request and receive notifications from Stamford Health on their attributed patients at no cost by submitting a patient roster either directly or through a PAC EHR integration. Providers will be able to select from flexible notification delivery options to suit their workflows including web application, SMS/email push alerts, and data downloads.
