Electronically

Call
You are also welcome to call the office for the provider you seek to refer to directly. Phone numbers are listed on each provider's Find a Doctor profile. Please plan to provide the minimum criteria when calling in your referral.
Fax
All provider offices have a dedicated fax number. You can use our Find a Doctor feature to find the fax number for the particular provider you intend to refer to.
- Please ensure any referral faxed over includes the minimum criteria to ensure your referral is able to be handled as quickly as possible.

What Patient Information Is Needed To Submit A Referral:
- Legal name (required).
- Date of birth (required).
- Phone number (required).
- Reason for referral (required).
- Medical reports as it relates to the referral (including pathology, labs, and radiology scans).
What Physician Information Is Needed:
- Provider full name and national provider identifier (NPI) (required).
- Name of practice.
- Practice address (required).
- Practice phone number (required).
- Practice fax number.
- Physician or practice e-mail.
