Choose Expert, Compassionate Care

When it comes to selecting the best primary care provider (PCP) for you and your family, Stamford Health’s physicians are dedicated to each patient's wellness. Your PCP treats you when you’re sick and provides preventative care so you can lead the healthiest lifestyle possible.

Our board-certified physicians have specialty training and are experts in diagnosing and treating simple, complex, and chronic conditions. Stamford Health Medical Group employs primary care, internal medicine, and family medicine physicians.

Your PCP serves as your health care partner and leads your medical team. When needed, they connect you with the right specialists, at the right times, and communicate with all your other physicians to ensure a coordinated care plan.

With a compassionate and informed approach, your PCP delivers preventative care so that you are up to date on screening tests and immunizations.

When You Need A Last-Minute Appointment

When you’re sick and need to see a provider quickly or before your PCP has availability, our walk-in centers provide flexibility for new and established patients.

Some benefits of our walk-in centers include:

  • Extended & Weekend Hours
Our walk-in centers and Immediate Care Center offer extended hours during weekdays and weekend hours to meet your health care needs.
  • Full Access To Your Health Records
If you’re an established patient, providers at our walk-in centers can view your medical history so they may treat you more effectively.
  • Shared Information With Your PCP
After you’re seen at a walk-in center, all your visit details are sent to your Stamford Health PCP for seamless follow-up care. If you do not have a Stamford Health PCP, a referral will be made for you.
  • Vaccine Appointments
When you receive a vaccine at our walk-in centers, your PCP is immediately notified.

Patient-Centered Medical Home

Stamford Health Medical Group is designated as a Patient Centered Medical Home (PCMH) for our all primary care and pediatric locations. This means it will be easy and comfortable for you to get the care you need, the way you want it. PCMH is a health care delivery model in which your primary care physician and a core team of caregivers manage your individual needs. Think of it as a support network, extended.

Virtual Care Options

If you need to see a doctor now, the Stamford Health On-Call Care team can provide the care you need 24/7, without an appointment. Download the On-Call Care app to find a provider.

If you’re an established patient, you can schedule a virtual care appointment with one of our Stamford Health Medical Group primary care physicians during office hours.

Primary Care FAQs

  • WHAT DO YOU NEED TO DO TO PREPARE FOR AN UPCOMING APPOINTMENT?
As a person-centered organization, we value patients as a key member of the care team. We welcome you to bring a care partner (spouse, family member, friend, or aide) with you to any upcoming appointments. Please ask questions, take notes, and let your care team know in advance if you need any special accommodations, including the assistance of an interpreter. For questions about how early to arrive or what forms you need to fill out, learn more on our Preparing For Your Doctor’s Appointment page. We look forward to welcoming you soon!
  • DO YOU NEED TO BRING YOUR MEDICAL RECORDS TO THE APPOINTMENT?
If you’re already an established patient within the Stamford Health network, there is no need to bring your medical records to an appointment. If you’re a new patient, please contact the practice before your appointment to understand if they’d like access to your previous medical records.
